Hello, this is Dr. Kim Ju-won, the chief director of Melon Plastic Surgery. Recently, I was selected as a presenter for APS KOREA 2025, an international academic conference on aesthetic plastic surgery co-hosted by the Korean Society for Aesthetic Plastic Surgery (KSAPS) and the Korean Association of Plastic Surgeons (KAPS), and had the opportunity to present a paper.
APS KOREA, which began in 1985, is celebrating its 40th anniversary this year, making it a deeply traditional academic conference. Sharing my expertise as a representative presenter was a very meaningful experience. The presentation topic was ‘Breast Augmentation for Asymmetric Patients Using Motiva Implants,’ and today I would like to share the content of that presentation with you.
Understanding and Approaching Breast Asymmetry

Breast asymmetry is a common phenomenon, observed in approximately 25% of the total female population. Asymmetry can naturally occur during the growth process, but it can also arise from various causes such as pregnancy and childbirth, breastfeeding, aging, and hormonal changes.
Asymmetry types are categorized into congenital, developmental, and acquired asymmetry. Understanding and approaching the characteristics and causes of each type is crucial for determining the treatment direction. The reasons for considering surgery are also diverse. In addition to aesthetic concerns, functional and psychological reasons, such as discomfort when wearing a bra or the desire to restore self-esteem, often play a complex role.
Pre-operative Consultation – Meticulous Checks are Key

When preparing for surgery in an asymmetric patient, the first step is to meticulously check the physical conditions.
In particular, breast width, chest wall structure, N-IMF distance (distance from nipple to inframammary fold), and soft tissue thickness are all important factors to carefully examine. While the width of each breast is often similar, a difference of 1-2 cm can occur, in which case careful consideration must be given to implant selection.
Asymmetry of the chest wall, especially in cases of scoliosis or rib asymmetry, can affect breast shape, requiring an even more delicate approach. This is why a design that considers the overall volume and silhouette of the breast, based on the nipple position, is necessary.
Reasons for Choosing Motiva Implants

Selecting implants for asymmetry correction is a truly crucial part.
While various brands are used, Motiva implants are often chosen. This is because Motiva is subdivided into four types: mini, demi, full, and corsé, and thanks to its regular diameter increase per size and abundant selection (approximately 80 types), surgical plans can be refined to suit each individual's physical condition.
Especially in surgery for asymmetric patients, where different types and volumes of implants are often used for each breast, Motiva's wide range of choices is helpful. The ability to select various types with the same diameter is also an important advantage in correcting asymmetry.
Sharing Surgical Cases of Asymmetric Patients

During this presentation, actual cases were also introduced.
Case 1 involved a patient with a sunken right chest wall and different left and right inframammary fold lengths. Surgery was performed using a Motiva Full 425cc implant for the right side and a Motiva Mini 260cc implant for the left side.
In Case 2, the left breast had a larger volume than the right, so a larger implant was inserted into the right side to achieve balance.
After observing the progress for 6 months to over 1 year post-surgery, both cases showed satisfactory results in terms of shape and symmetry maintenance.
Asymmetric breast augmentation is a surgery that requires delicacy.
I believe that understanding and respecting each individual's physical characteristics and psychological needs is paramount. What I realized while preparing for this APS KOREA presentation was the importance of never losing the attitude of learning and researching, no matter how much experience one has accumulated.
